Menstrual Suppression Tips

First go to is Primolut 5mg BD - usually stops bleeding and is flexible in dosing regime. Can go up to 10mg TDS daily and then slowly wean down to lowest dose needed (5mg daily). Use every day and benefit of not having oestrogens. Remind patient that it is not a contraceptive and even if they do not get periods, they can still get pregnant.

 

COPC works well too if young person is ok with oestrogens and they might also desire contraceptive effect. Run pill packs back to back and avoid inactive pills. Can get breakthrough bleeding and might need a break every few months to get a bleed and start again.

 

Mirena IUS - good option for combined reduction in bleeding as well as contraception, "set and forget" for 5 years. Can be very dysphoric to talk about uterus etc, but some young people are open to this option and it's very safe and rapidly reversible.

 

Depo-provera - q3mo injections. These impact bone density so if not needed for contraception there are generally better options, but works very well to suppression HPO axis and stop periods. Takes 3 mo to get out of system so not quickly reversible

 

Puberty suppression with GnRHa - a bit of overkill but if other means not working and high degree of distress, can be considered but not ideal due to impact on bone density.
